COURSE REGISTRATION FORM
Instructions: The form above should be used to register for a Metaplexity Associates course. Fill out the form as described below. Each participant from your organization must complete and submit a form. Upon completion of the required fields, accept the terms below by checking the box near the Register button and send the form to us via email by pressing the Register button. The Register button will not work unless the terms are accepted.
Contact Information: It is very important to provide accurate information. Most of our courses award certification in the subject and the information you provide is submitted to the certifying authority, usually The Open Group. Mistakes can be difficult to correct once information has been entered into their certification registry.
Course Information: Select the type of course, starting date and location. Enter the amount of the Course Fee including any promotional codes that you have received.

Payment Information: Once your registration form has been received with the Course Fee amounts as quoted to you, you will receive a PayPal invoice which may be paid with your payment method of choice, including VISA, Master Card, American Express, and Discover cards, or other arrangements as you might have with PayPal.
If a company purchase order is selected and has been approved as a form of payment, please indicate the company information needed to process the purchase order. We may need to work with you to get set up in your company’s vendor payment system and/or to be set up for ACH payments.
All payments must be received before attending class unless previous arrangements have been made.

Course Cancellation Policy
Public Course Cancellation Policy
If the registrant cancels, in writing, more than ten (10) business days prior to the starting date of a course, the registrant will receive a 100% refund.
If the registrant cancels, in writing, within ten (10) business days, registrant agrees to pay Metaplexity Associates, Inc. a cancellation fee equal to twenty percent (20%) of the registration cost for each registrant who cancels.
If the registrant fails to cancel, in writing, and doesn’t attend courses, registrant agrees to pay Metaplexity Associates Inc. a cancellation fee equal to one hundred percent (100%) of the registration cost for each registrant who failed to attend courses.
Public Course Rescheduling Policy
If the registrant reschedules for a future course date, in writing, more than ten (10) business days prior to the starting date of course, the registrant will not be assessed rescheduling fee.
Any registrant who reschedules within ten (10) business days before the starting date of a course will be charged a twenty percent (20%) rescheduling fee, based on the price of registration.
Unless other arrangements are made by mutual agreement, course and examination fees are due to be paid in advance of the course start date. If paying by purchase order, payment is due upon submission of an invoice for applicable fees. For direct payments, we will send to each registrant a PayPal invoice upon receipt of their completed registration form, and the enrollee will, upon successful processing of payment, be issued a receipt for the training costs.
